2. Copy of Parent’s ID (KTP / SIM / Passports for Expats) 
3. Copy of Birth Certificate (Student). Please provide English translations if it is not in Bahasa Indonesia, English
4. Copy of Immunization Record
5. Blood Group 
6. For Expatriates : Copy of KITAS 
7. For Indonesian Family : Copy of Family Register (Kartu Keluarga ) 
8. Copy of Student’s Progress reports / report card / last academic record from previous school (endorsed copy from the principal or relevant school authorities)
9. School leaving certificate from previous school 
10. Recent  passport size photo (3 x 4; 4 pieces or 2 x 3 1 piece black & white)
